Medical/Dental/Vision:

After 6 months, on the 1st day of the following month, employee will be covered.
Any employees wanting coverage for dependents will be required to pay for those
dependents. Our Medical/Dental/Vision provider is Regence BlueShield and you can
view our policy here.

401(k) program:
|
After 6 months employee is eligible for the program and can receive all the information
about the program from Human Resources.

Aflac:
|
After 6 months employee is eligible for the program and can receive all the information
about the program from our AFLAC representitive, Kristen Conrad at (253)-226-5750
